SEVP has made available this fax cover sheet, to be used when a PDSO chooses to submit required supporting documentation to SEVP by fax, rather than by e-mail, when requesting an adjudicable change to the school's PDSO or DSOs. The fax cover sheet was included as an attachment to SEVP Broadcast Message 1108-01, sent August 8, 2011.
